PROACTIVE COMMUNITY SERVICES (PCS), a nonprofit organization in the Southern Cook County Suburbs, provides services to educate and bring awareness to combat social and health disparities. PCS aims to empower individuals and encourage behaviors that promote long-term healthy lifestyles, economic self-sufficiency, and independence through individual achievement. Our team is passionate about creating positive change in our community and beyond. We are seeking to fill our Housing Technician position.
POSITION DESCRIPTION:
Ability to provide housing information and linkage services to a diverse set of households impacted by chronic medical issues
Ability to respectfully interact with clients and landlords
Ability to complete home visits to clients’ homes as necessary
Ability to work cooperatively as part of a team
Ability to use databases.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Complete required reassessment and recertification documents for households in the Trust Fund and Long-Term Rental Assistance programs and submit them to the AIDS Foundation of Chicago (AFC) and Trust Fund as required.
Participate in bimonthly coordination and training meetings at AFC.
Maintain confidential records of clients served and submit mandatory reporting requirements.
Document all client and client-related services in the corresponding database(s). Advocate for clients with landlords and providers as needed.
Assist clients in the community (non-LTRS or Trust Fund) on a short-term basis to obtain and maintain safe, affordable housing and referral to support services.
Serve as a housing resource for Ryan White Medical case managers throughout the EMSA.
Develop relationships with landlords to facilitate client placement.
Provide tenancy support, including financial counseling and education on tenant’s rights and responsibilities.
Maintain client files in accordance with AFC and Chicago House policies. Participate in the Housing Inquiry Line.
Attend and participate in appropriate staff meetings
Report clients’ health and/or behavior changes to the supervisor. Perform other duties as assigned
Requirements
REQUIREMENTS/QUALIFICATIONS:
High School Diploma or GED required; Bachelor’s degree preferred. Demonstrated excellent oral and written communication skills.
Team-oriented work style.
Knowledge of local housing resources and program requirements preferred. Knowledge of tenancy rights and landlord-tenant laws preferred. Bilingual/bicultural preferred.
