This position is responsible for directly supporting clients with daily rehabilitation goals while managing staff scheduling grids, training direct care staff, and ensuring client treatment strategies are consistently carried out—with a primary focus on evening and weekend shifts.Key ResponsibilitiesDirect Client Support & Care Implementation

Direct Care & Daily Living: Provide minimum to maximum assistance or supervision to promote client success in daily living tasks, hygiene routines, meal planning/prep, apartment upkeep, budgeting/money management, pedestrian safety, and recreational or vocational activities.Rehabilitation Exercises: Assist clients with physical therapy (PT) routines, visual exercises, walking programs, and transportation training according to their individual treatment plans.Behavioral & Physical Health Monitoring: Recognize changes in clients' physical, medical, emotional, cognitive, or behavioral status and immediately report changes to the clinical team. Skillfully support clients through challenging situations (e.g., activity, strategy, or medication refusals).Core Care Competencies: Maintain proficiency in medication administration, safe client transfer techniques, medical procedures, job coaching, and client transportation.

Staff Training & Clinical Carry-Over

Training & Mentorship: Train, observe, and mentor facilitators to ensure client strategies and systems are correctly maintained across all shifts.Onboarding & Annual Compliance: Coordinate shadowing schedules for new hires and oversee required annual staff observations (such as DPW Medication Observations).Cross-Team Collaboration: Work closely with Lead Facilitators, Clinical Specialists, Site Managers, and therapists to evaluate, implement, and give feedback on treatment plan effectiveness.

Operations, Grid Management & Scheduling

Master Grid Management: Build, manage, and modify weekly staff work grids and client activity schedules using Microsoft Excel and internal client information systems.Efficiency & Budget Optimization: Integrate therapist, nursing, RCM, and facilitator schedules to maximize direct care hours while adhering strictly to budgeted hours set by the Operations or Site Manager.Quality Assurance (QA): Verify that client weekly schedules align with staff work grids and conduct routine checks to ensure staff follow through on grid assignments.Emergent Schedule Modifications: Adjust daily staff grids (day and evening shifts) in response to call-outs, client appointment changes, or emergency staffing needs.Resource Coordination: Coordinate house/apartment staff, vehicles, and transportation schedules for maximum operational efficiency.

Qualifications & Requirements

Education: High school diploma required; Associate's or Bachelor's degree in Healthcare, Human Services, or Business Administration preferred.Experience: 3 to 5 years of healthcare experience with strong administrative and office management skills. Experience in neurological rehabilitation or residential care is strongly preferred.Technical Skills: Advanced proficiency in Microsoft Excel, database entry/management, word processing, and electronic scheduling systems.Key Competencies: Exceptional time management, organizational skills, problem-solving ability, and clear communication with multi-disciplinary care teams.Licensure & Driving Requirements: Valid driver's license with a record of responsible driving is required.
